Do most g-code files set a project "zero" from the machine's normal "home"?

In other words, do most g-code files EXPECT the CNC head to be PRE-positioned at the machine zero? (x/y in upper/left when looking down).

Or do most expect you yourself to manually set the position of x/y to what you "think" should be the starting position for the project/cutting and then set THAT manual staring location as home via  the gcode G92 or G52/G54 commands?
